ICD-10 Code H05.4 – Enophthalmos (2026): Diagnosis, Symptoms & Billing Guide
2026 ICD-10-CM Diagnosis Code H05.4 – Enophthalmos
What it is
Enophthalmos means the eyeball sits farther back in the orbit than normal. You use this code when documentation identifies posterior displacement of the eye, usually as a clinical finding rather than a diagnosis by itself.
Clinical signs
Typical findings include a sunken-appearing eye, reduced forward projection, and facial asymmetry. Clinical features vary; refer to documentation for the underlying cause and whether the finding is unilateral or bilateral.
When to use this code
Use H05.4 when the provider documents enophthalmos on exam, imaging, or consultation notes. It may appear with trauma, orbital fracture, volume loss, or other orbital disorders, but you should code the documented finding as written. If the cause is stated, code that condition as well when appropriate.
Do not use for
Do not use this code for generic eye pain, swelling, or other orbital abnormalities without documented enophthalmos. Check documentation if the note describes proptosis, because that is the opposite finding.
Coding tip
Confirm whether enophthalmos is a current finding, a history item, or part of a broader orbital diagnosis before assigning H05.4.
